| Answer To Question 1: The team utilise paper proformas. | |
| Question Number 2: If your Trust uses paper systems, how are these transcribed into electronic systems. | |
| Answer To Question 2: The proformas are scanned into the digital system, Badgernet. | |
| Question Number 3: If your Trust uses an electronic scribing or documentation tool within your unified electronic patient record (EPR), please confirm whether this tool is used to document obstetric emergencies in real time as the event unfolds, or whether documentation is typically completed retrospectively after the emergency has concluded. | |
| Answer To Question 3: Maternity use Badgernet for documentation but NOT for emergencies, paper proformas are utilised. The information is not transcribed into the electronic records but scanned as a real time working document. | |
