| Answer To Question 6: Response from Medicine : 6. ED Trackers Division: Medicine. Department: ED Operational Managers Division: Medicine. Department: ED/AMU SPOC/Silver Bleep holders Across all divisions: Medicine, Surgery, FASS and Paediatrics. There are specific flow roles at SFT – Head of Site and Patient Flow, Matron for Flow that cover patient journeys through the hospital from ED to Discharge. Divisions have roles that are also designed to facilitate flow through the organisation, and there is a culture that it is everyone’s responsibility to ensure a smooth and effective journey through their inpatient stay. In ED there is the OPAL team – therapy assessment designed to explore quick support for people who don’t need to be admitted to secondary care and could return home with therapy/equipment /rapid response care support. Wiltshire has a hospital at home service via HCRG which supports people at home rather than be admitted to hospital – further details available from the provider. Wiltshire has a care coordination centre for health professionals to call, including ambulance crews, to explore alternative options for patients in the community who may not need to attend ED if appropriate services are put in place in the community. This service is not owned by SFT and so enquiries to BSWICB re service outline are recommended. | |

| Answer To Question 7: Response from Medicine: Yes. Patients are seen in the ED, and if appropriate may be placed on a clinical pathway and await results in a clinical decisions unit which leads to reduction in admissions. Same day emergency care exists for medicine and Surgery, which aims to investigate/treat and discharge, again avoiding admission. | |
