| Question Number 1: You compile data from your trust which is then submitted and published in the Estates Return Information Collection (ERIC) data spreadsheet. - NOTE: If you had submitted data on more than one 鈥渕ost鈥� clinically impactful incident because you have a number of sites each with their own 鈥渕ost鈥� clinically impactful incident I would require you to answer these questions repeatedly for each site that you have responsibility for. One of the categories on this spreadsheet is 鈥淢ost clinically impactful estates incident鈥� and if you had such an incident you were then required to provide data on the type of incident, the cost to rectify it, etc. My question is if you reported on such an incident (I am only interested in the 鈥榤ost鈥� clinically impactful incident) please provide me with the following information: a. The dates when this incident occurred. b. Where it took place. c. A one sentence summary of what the incident was and how it impacted the hospital, and d. A copy of any report on the incident that was produced after the event to explain what the incident was and how it impacted the hospital/trust. This should be limited to just reports that were produced either by or for members of your Trust鈥檚 executive management board. e. If there were reports on the incident that do not fit into part 4 of my question then please just list the titles of these reports and the dates that they were published. | |
