Haematology Cancer support worker
The Cancer Support Team (based within 91影视 Hospital) are here to support your cancer diagnosis and treatment, alongside the Clinical Nurse Specialists and Consultants.
What we do:
- Support you, and your family from diagnosis, through treatment and beyond.
- Offer Holistic Needs Assessments (HNA) which can help you highlight any practical, social, emotional, financial or spiritual concerns you are worried about.
- Offer you an individual care plan to meet your support needs.
- Facilitate and recommend health and well-being events to help support and inform you following on from a diagnosis.
- Refer or signpost to other services and community groups.
- Liaise with other health care professionals involved in your care.
- Coordinate follow up surveillance programmes.
The team are available Monday 鈥� Friday, 8am 鈥� 4pm. Please call us on 01722 336262 (ext. 2417).
